گفت و گو با دکتر نیکبخت؛ پزشک کلینیک سایه.

سلام آقای دکتر، لطفاً خودتان را به‌رسم کنگره معرفی بفرمائید:

سلام مهران هستم یک همسفر و پزشک متخصص عفونی هستم.

لطفاً از نحوه همکاری‌تان با کنگره 60 و آشنایی‌تان با کنگره 60 بفرمائید.

من از سال 1389 با آقای مهندس دژاکام آشنا شدم، آن موقع پزشک عمومی بودم و در حال حاضر متخصص بیماری‌های عفونی و پزشک بیماری‌های عفونی در بیمارستان مرکز قلب تهران، بیمارستان رازی و آرش تهران هستم. در آن سال من پزشک عمومی بودم و یک درمانگر اعتیاد بودم و الآن هم خودم را درمانگر اعتیاد می‌دانم. در آن زمان وزارت بهداشت در حال بهره‌برداری از داروی جدیدی برای درمان اعتیاد بود و من یکی از اولین پزشکانی بودم که افتخار کار با این دارو را داشتم. با آقای مهندس به این واسطه آشنا شدم و این همکاری ادامه داشت که تاکنون ادامه پیداکرده است.

آقای دکتر، آمار حدودی از رهجوهای کنگره 60، از بدو ورود تا به الآن و عزیزانی که به رهایی رسیده‌اند، بدهید:

من جزو اولین کلینیک‌هایی بودم که افتخار این را داشتم که با کنگره همکاری داشته باشم، فکر می‌کنم در این 10 سال، تقریباً نزدیک به 1000 نفر آمار ورودی از کنگره 60 داشته‌ایم و حدود 600 تا 700 نفر به رهایی کامل رسیده‌اند. تمام این آمارها به جزئیات قابل‌پیگیری است.

آقای دکتر، نظرتان در مورد تئوری ایکس آقای مهندس دژاکام و متد دی اس تی در درمان اعتیاد چیست؟

این دو واژه در ابتدا واقعاً واژه‌های عجیبی بودند. ما در ابتدا ذهنیت خوبی از صحبت‌های کنگره و آقای مهندس نداشتیم، آقای مهندس، پیچیدگی‌های بدن انسان را یک ایکس فرض می‌کردند و هر آنچه این سیستم و بالانس طبیعی را به هم می‌ریزد، به اسم آنتی ایکس نام ‌برده بودند. درواقع مواد را آنتی ایکس معرفی کرده‌اند؛ مثل حشیش، تریاک و ... . این مواد به سیستم فیزیولوژی بدن آسیب می‌زند و برای اینکه این ایکس به تعادل برسد، ادعا کردند که باید از متد دی اس تی استفاده شود. این مطالب را ما در طی حضورمان در کنگره آموختیم.

نظرتان راجع به داروی اپیوم تینکچر و تأثیر آن در درمان بیماری اعتیاد، نسبت به سایر داروهای موجود در بازار بفرمائید:

معرفی اپیوم تینکچر به عنوان یک داروی جدید درمان، یک اقدام جالب بود که توسط وزارت بهداشت انجام گردید. آقای مهندس هم جزء افرادی بودند که پیشنهاد تولید این دارو را دادند. این دارو ارزشمند بود، ابتدا ما این دارو را نمی‌شناختیم. در آن زمان متادون در کنار بوپره نورفین خیلی مرسوم بود. هرچه زمان بیشتری گذشت، ارزش این دارو بیشتر پیدا شد و دلایل موفقیت این دارو تا به الآن، این بود که تعادل بهتری در درمان اعتیاد ایجاد می‌کرد، وابستگی کمتری داشت، راحت‌تر کم می‌شد، بالانس فیزیولوژیک بهتری را ایجاد می‌کرد.

با توجه به فرمول ساده آن (آب و الکل به مقدار کم و اپیوم) سال‌های اول که این دارو تولید شد، در مرکز مطالعات ملی ایران آزمایش شد و نتایج خیلی درخشانی داشت. الآن هم با کمک آقای مهندس و وزارت بهداشت، نتایج خیلی خوبی از این دارو گرفتیم. تأکیدی که من دارم، این است که به همه‌ی این‌ها باید به دید یک دارو نگاه کرد و اگر از همه‌ی این داروها به‌درستی استفاده نشود، نه‌تنها اثرات مؤثری نخواهد داشت، بلکه اثرات مخربی هم خواهد داشت.

آقای دکتر، به نظر شما مهم‌ترین تفاوت‌های درمان شدگان کنگره 60، با افرادی که به روش‌های دیگر و داروهای دیگر اقدام به ترک می‌کنند، چیست؟

برای من که سال‌ها از نزدیک با بچه‌های کنگره 60 کارکرده‌ام و آن‌ها را می‌شناسم، این تفاوت از آسمان تا زمین است. تفاوت‌های عمیق و زیادی بین این روش درمان و داروی آن و سایر روش‌ها وجود دارد و یک دنیا متفاوت هستند.کسانی که در کنگره پرورش پیدا می‌کنند، از ابتدا گروهی کار می‌کنند. این گروه یک مجموعه بیمار محور است و نگاه مهندس هم به اعتیاد این است که اعتیاد یک بیماری است. در طول مسیر درمان یک کمک راهنما که خود قبلاً مصرف‌کننده مواد مخدر بوده است و هم درد آن‌ها بوده است، بیماران را همراهی می‌کند. در درمان کلینیکی، مخاطب بیمار معمولاً پزشک یا روانشناس است که اعتیاد نداشته و شرایط را لمس نکرده است، ولی در کنگره این‌گونه است که به کمک یک فردی که قبلاً اعتیاد داشته و این مسیر را طی کرده است، درمان می‌شود. در کنگره، مسیری که برای درمان اعتیاد طی می‌شود، سفر اطلاق می‌شود.

نکته مهم‌تر در کنگره 60 که گروه‌های درمانی دیگر از آن بهره‌ای نبرده‌اند، داروی درمان و همان داروی اپیوم تینکچر یا اوتی است. این دارو در کنار روش درمانی کنگره، به آن‌ها کمک بسیاری می‌کند. دریافت داروی این افراد، کاملاً وابسته به حضور در این جلسات است و اگر در جلسات حضور نداشته باشند و برنامه را درست اجرا نکنند، دارویشان قطع می‌شود. درواقع، در این روش روی باز توانی بیماران کار می‌کنند. در کنار آن با ورزش کردن روی بازسازی جسم کار می‌کنند و برای بازسازی روان و تغییر جهان‌بینی، آموزش‌های لازم را به بیماران می‌دهند. یکی از تفاوت‌های فاحش، ایجاد تعادل است. کسانی که با این متد شروع به درمان می‌کنند، خیلی سریع به آن تعادل لازم می‌رسند. مجموعه‌ی چندین فاکتور این اتفاق بزرگ که همان درمان قطعی است، می‌شود.

نظرتان در مورد حضور پزشکان در پارک طالقانی و تأثیر آن روی شما و مسئولین کلینیک‌ها چیست؟

تشکیل لژیون پزشکان در پارک، به پیشنهاد خود بنده و آقای مهندس در پارک طالقانی انجام شد. به خاطر اینکه ما جمعه‌ها خدمت آقای مهندس می‌رسیدیم و در آنجا با مسائلی که اصلاً برای ما قابل‌لمس نبود برخورد می‌کردیم. لذا به پیشنهاد خود بنده، برای اینکه با مفاهیم کنگره و آمیزه‌های کنگره آشنا بشویم، آقای مهندس محبت کردند و پذیرفتند که لژیون پزشکان تشکیل شود. الآن لژیون پزشکان یک ظاهر حرفه‌ای دارد، ولی 10 سال پیش، یک تکه سنگی بود که آقای مهندس به آن تکیه می‌دادند و پزشکان دور ایشان جمع می‌شدند و آموزش‌ها داده می‌شد. اولین دبیر آن جلسات در لژیون پزشکان اتفاقاً خود بنده بودم و بدین ترتیب لژیون پزشکان شکل امروزی را گرفت.

آقای دکتر لطفاً در رابطه با تشکیل لژیون درمان اعتیاد کنگره 60 در داخل کلینیک و تأثیر آن توضیح بدهید.

عده‌ای از بیماران که در کنگره نبودند؛ اما از کلینیک دارو می‌گرفتند متقاضی این بودند که با متد و شرایط کنگره به درمان برسند، آقای مهندس هم موافقت کردند که لژیون‌های کنگره در داخل کلینیک‌ها برقرار شود. من چون سال‌ها درگیر دریافت تخصص پزشکی خود بودم، نسبت به سایر همکاران، عملکرد ضعیف‌تری داشتم، ولی خدا رو شکر الآن شرایط خیلی بهتر شده است و بچه‌ها بر طبق آموزش‌های کنگره آموزش می‌بینند و به درمان می‌رسند.

آقای دکتر، کلام آخر

از شما و کنگره 60 و آقای مهندس دژاکام تشکر می‌کنم که این فرصت را در اختیار من قراردادند تا بتوانم تاریخچه حضور در کنگره 60 را خدمتتان مطرح کنم. امیدوارم این مسیر و تلاش برای رهایی انسان‌ها ادامه داشته باشد. هر یک‌نفری که بتوانیم به رهایی برسانیم، یک خانواده را به رهایی رسانده‌ایم که ارزش بسیار بالایی دارد و اصلاً قابل‌شمارش نیست. امیدوارم این همکاری بین همه کلینیک‌ها و کنگره وجود داشته باشد و قدر این روش درمان را بدانند و برای استفاده از این سیستم روزبه‌روز امکانات و شرایط مهیاتر باشد.

در آخر هم ما باید قدر این شرایطی که به وجود آمده است، بدانیم. ممکن است نواقصی وجود داشته باشد؛ اما بایستی مطالعات و بررسی‌های دقیق‌تری انجام بدهیم و هرچه می‌توانیم این سیستم‌ها را به‌روزرسانی کنیم. من بشدت از روزی می‌ترسم که کار در دست افراد کاردان نباشد و روش‌ها و پیشنهادات اشتباهی در این حوزه توصیه شود. توصیه می‌کنم از تجارب دوستان پیشکسوت من در این فیلد بیشتر استفاده بشود. باید سیستم موجود را تا می‌توانیم تقویت کنیم تا استفاده خیلی بهتر و خدمت‌رسانی بهتری داشته باشیم. کشور ما کشور آلوده‌ای ازنظر مصرف مواد مخدر است و اگر سیاست‌گذاری درستی انجام نشود و به‌درستی در این مسئله گام برنداریم، می‌توانیم تمام زحمات سال‌های گذشته را نابود کنیم.

Hi, I am Mehran a companion.

Please tell us about your acquaintance with Congress60 and your cooperation with this NGO.

I have met Mr. Dezhakam since 2010 when I was a physician. Currently, I am an infectious disease specialist and I am working in Tehran Heart Center Hospital, Razi, and Arash Hospital. Those years when I met Mr. Dezhakam I was a general practitioner and an addiction therapist. I have to say, I still know myself as an addiction therapist. At that time, there was a cooperation between the Health Ministry of Iran and Mr. Dezhakam to exploit a new medicine for addiction treatment and I was one of the first physicians who had the honor of working with this group. I got to know Mr. Dezhakam through this project and this cooperation has continued until now.

Can you give us a rough number of patients who have been treated in your clinic?

My clinic was one of the first clinics that cooperated with Congress60. We had approximately 1000 patients from Congress60 in 10 years and between 600 to 700 of them have been completely liberated from drugs. All of these numbers can be tracked in our documents.

What is your opinion about the X-theory and DST method?

At first, both of these terms were quite strange for us. We had not a good mindset about what Congress60 and Mr. Dezhakam were speaking about. Mr. Dezhakam thought of the complexities of the human body as an X, and anything that disrupted this system and its natural balance was called anti-X.
In fact, drugs (like Cannabis, Opium, etc) have been introduced as anti-X. These drugs damage the body's physiological system. He claimed that in order for the X system to reach equilibrium, the DST method had to be used. We have learned this during our presence in Congress60.

What do you think about opium tincture syrup and its effect on addiction treatment compared to other drugs on the market?
 

The introduction of opium tincture as a new medicine for addiction was an interesting step taken by the Ministry of Health. Mr. Dezhakam was one of the people who suggested the production of this drug. At first, we had not a clue about this valuable medicine. Although, Methadone and Buprenorphine were very popular those days, the longer it took, the more valuable this drug (Opium tincture) became. Reasons behind the success of this drug were, striking a better balance in the treatment of addiction, less dependence, comfortable tapering off, and better physiological balance.

 Due to its simple formula (water, alcohol, and opium) in the first years that this drug was produced, it was tested in the National Studies Center of Iran and had very brilliant results. Now we have gotten very good results from this drug thanks to Mr. Dezhakam and the Ministry of Health.

 My emphasis is that we have to consider all drugs which are used for addiction treatment as medicine and if all of these drugs are not used properly, not only they will not have effective effects, but they will also have destructive effects.

In your opinion, what is the main difference between those who are treated in Congress60 with opium tincture syrup and other people who are treated by other drugs?

For me, as a person who has worked closely with the Congress60’s members for many years and knows them, this is the difference from heaven to earth. There are many profound differences between this method and its medicine and other methods. Those raised in Congress60 learn how to work as a team. Congress60 is a patient-centered group, and Mr. Dezhakam sees addiction as a disease.

Throughout the treatment period, a guide-assistant who used to be an addict accompanies patients. In clinics, patients should speak with a physician or a psychologist who was not an addict and has not touched addiction personally. However, but in Congress60, patients can use the help of someone who is already addicted and has gone through this process.

In Congress60, the path to addiction treatment is called a journey. The most important thing in Congress60, which has not been used by other treatment groups or NGOs, is opium tincture or OT which plays an important role for the treatment process. This drug, along with their treatment method (working on their worldview by training them), helps them a lot.

 Receiving patient’s medicine is completely dependent on attending meetings, and if they do not attend the meetings and do not implement the program properly, their medicine will be stopped. In fact, they rehabilitate patients.

 Besides, they reconstruct patients’ physiology by doing sports, and by giving the necessary training and knowledge to patients help them to mentally rebuild and change their worldview.

Their other difference is balancing. Those who start treatment with this method will reach the necessary balance very quickly. The accumulation of these factors leads to definitive treatment.

What is your opinion about the formation of physicians’ legion at Taleghani park and its impacts on physicians and clinic managers?

The formation of the physicians' legion in the park was done at the suggestion of me and Mr. Dezhakam in Taleghani Park. Because we used to come to Taleghani Park on Fridays and we had to deal with issues that were not tangible for us at all. Therefore, Mr. Dezhakam accepted my recommendation to get acquainted with Congress60’s concepts and knowledge.

Now the physicians’ legion has a professional appearance and a place, but 10 years ago, it was holding near a piece of rock on which Mr. Dezhakam leaned and doctors were gathering around him to receive training. I was the first secretary of this legion and thus the physicians’ Legion took on its present form.

 Please explain a bit about the formation of Congress60’s addiction treatment legion in your clinic and its impact on the clinic's performance.

Some of the patients who were not Congress60’s members but were taking medicine from our clinic asked us to be treated according to the methods and conditions of Congress60, so Mr. Dezhakam approved legions to be held on clinics. Because I have been involved in receiving my medical specialty for years, I have performed worse than my other colleagues. However, the situation is much better now and patients are being trained and treated according to Congress60’s method.

I would like to thank you and Mr. Dezhakam for giving me this chance to give you a brief history of my presence in Congress60. I hope this path continues for human beings to liberate them from addiction. Every single person we can liberate is like we have liberated a family that is of great value and cannot be counted at all. I hope that the cooperation between all clinics and Congress60 reaches a high level, and the conditions and facilities will be more available to expand this knowledge so that they will use this treatment method more than before. Finally, we must appreciate these conditions that have arisen. There may be shortcomings, but we need to do more careful research and update these systems as much as we can. I am very afraid of the day that this work not to be in the hands of skilled people and the wrong methods and suggestions alternate this path to the wrong path. I recommend making more use of the experiences of my friends in this field. We need to support this system and strengthen it so that we can have much better use and service.

 Our country is polluted in terms of drug use, and if the right policy is not done and we do not take the right steps in this regard, we can destroy all the past efforts.
