همیشه شنیدن از شما و آخرین اخبار کنگره ۶۰ خوشایند است. همچنین از جواب دادن به سؤال‌هایم واقعاً ممنونم.

همواره مراسم سالانه کاشت درخت برای من بسیار جالب بوده است. به جز اینکه این مراسم از نظر شهروندی و اجتماعی خدمتی ارزنده محسوب می‌شود، مراسمی فوق العاده است که بازسازی و دوباره‌سازی که همراه ریکاوری از اعتیاد است را به نمایش می‌گذارد.

تعداد کلینیک‌هایی که در ایران از متد دی اس تی استفاده می‌کنند چه خبر خوبی است. نگرانی من این است که کلینیک‌ها در آمریکا تنها از جنبه‌ی دارویی متد دی اس تی استفاده کنند و حمایت‌های وسیع ریکاوری آن‌که قسمت بسیار مهمی از کنگره ۶۰ است را نادیده بگیرند. ما اکنون درمان دارویی با بوپرنورفین در کلینیک‌ها داریم که خدمات بسیار کم روان‌شناسانه هم برای ریکاوری بلندمدت از اعتیاد ارائه می‌کنند.

منتظر عکس‌های شما از مراسم رمضان هستم و چقدر خوشحالم که امسال این مراسم دوباره برگزار می‌شود. با کم شدن اعداد مربوط به مبتلایان به کرونا در سرتاسر جهان بسیار خوب است که دوباره مراسم مهم برگزار می‌شود و مردم گردهم می‌آیند. 

جرج از مکاتبات با شما بسیار راضی است. کاری که کنگره ۶۰ در رابطه با سیگار کرده است برای جرج بسیار الهام‌بخش بوده است و او درباره اهمیت قطع سیگار در جوامع ریکاوری مقاله می‌نویسد و اکنون با گروه دیگری که زیرمجموعه جامعه اعتیاد دارویی آمریکا هستند درباره قطع سیگار همکاری می‌کند. این‌یک گردهم آیی مهم برای گسترش کار کنگره ۶۰ خواهد بود.

بابت پاسخ به سؤال‌های اخیر ممنونم. این سؤال‌ها که ارسال می‌کنم رایج‌ترین سؤال‌هایی است که از خوانندگان مطالب مربوط به کنگره ۶۰ دریافت می‌کنم. این پاسخ‌های شما کمک می‌کند تا جوابی برای آن‌ها داشته باشم.

در مکاتبات قبلی گفته بودم که شرایطی مهیا می‌کنم که نگهداری از سایت من به سیستم سلامت چست نات سپرده شود. من در سال ۱۹۷۳ جزو بنیان‌گذاران این مجموعه بودم و بعداً دوباره به آنجا بازگشتم و قسمت تحقیقات و تمرین آنجا را به راه انداختم و ۲۵ سال از حرفه من در آنجا گذشت.

این انتقال برای این است که مطمئن شوم که وقتی دیگر نتوانستم از سایت نگهداری کنم بازهم این مطالب حفظ خواهند شد. کار تقریباً به اتمام رسیده و در ماه‌های آتی تغییرات کوچکی در وب‌سایت من خواهید دید. خبر خوب این است که مطمئن خواهیم شد که مطالب شما روی وب‌سایت من تا سالهای دور باقی بماند. احساس می‌کنم که این بخشی از میراث و هدیه ما به جهان خواهد بود.

در آمریکا علاقه به ترکیب درمان دارویی با درمان‌های روان‌شناختی رو به افزایش است. من با گروهی از رهبران این کار ملاقات داشته‌ام و برای آن‌ها توصیف کرده‌ام که این ترکیب چگونه در کنگره ۶۰ کار می‌کند. فکر می‌کنم این جلسات آتش علاقه به کنگره ۶۰ را شعله‌ورتر کند. جرج هم در این گروه حاضر است و او نیز منبعی خواهد شد تا ایده‌های کار شما را به روان‌پزشکان حوزه اعتیاد آمریکا انتقال دهد.

شاید قبلاً نوشته باشم که خواهر بزرگ من بعد از ۱۰ سال درگیری با آلزایمر درگذشت. این هفته به الینویس سفر می‌کنم تا در مراسم یادبود او شرکت کنم. او یک پرستار قسمت کودکان بود که زندگی خود را صرف کمک به کودکان مبتلا به بیماری‌های خطرناک کرد. انسان فوق‌العاده‌ای بود و دل‌تنگ او خواهم شد.

Dear Hossein,

It is always a pleasure to get your latest communication and to hear of the latest Congress 60 activities. And I do appreciate the care you take in responding to my questions.

I have always found the annual tree planting ceremony of great interest. Beyond being a wonderful act of citizenship and community service, it is such a wonderful ritual symbolizing the regeneration and renewal that accompanies recovery from addiction.

What good news you share on the number of clinics in Iran now using the DST method. My worry would be that clinics here in the U.S. would use the DST method as a medicinal cure but not adopt the larger recovery supports that are such an important part of Congress 60. We now how medication clinics (buprenorphine) that offer few if any psychosocial supports for long-term addiction recovery.

I look forward to your photos of the Ramadan ceremonies which I am delighted to hear will be revived this year. With coronavirus numbers declining in many countries, it is wonderful to see people being able to once again gather during such important times.

George is very pleased with your continued communications. What Congress 60 has done around smoking cessation has been deeply inspiring to George and he is writing articles on the importance of smoking cessation in addiction recovery and is also now working with a special interest group on nicotine addiction that is part of the American Society of Addiction Medicine. This will be an important forum to further disseminate the work of Congress 60.

Thank you for your responses to my most recent questions. The questions I send are ones I get most frequently from those who read my papers and our blogs on Congress 60. Getting these responses from you will help me respond to these queries.

I am delighted to hear that you will be piloting the DST method within homeless shelters for drug users. I look forward to hearing how well this works. I think that is an important outreach effort that could bring recovery to individuals experiencing severe addiction.

In earlier communications, I mentioned that I was arranging to have oversight of my website transferred to Chestnut Health Systems. I helped found that organization in 1973 and later returned to help found their research and training division and worked there the last 25 years of my career. The transfer of my website to their oversight is to make sure the resources posted there will be available after I am no longer able to maintain the site. That work is near completion so you will see a slightly different look to the website in the coming months. The good news is that this will assure that our papers and your writings will remain available on my website far into the future. I fell that is part of our legacy and gift to the world.

There is also growing interest in the U.S. in integrating medication approaches to treatment with treatment that rely exclusively on psychosocial support. I have been meeting with a group of leaders interested in such integration and I’ve described to them how such integration works within Congress 60. I think these discussion will spark greater interest in the work of Congress 60. George is part of this group and will also be a source for carrying ideas from your work to a U.S. audience of addiction psychiatrists.

I may have shared with you in my last communication that my oldest sister passed away after 10 years of decline from Alzheimer’s disease. I will travel to Illinois this week to attend a memorial service in her honor. She was a pediatric nurse practitioner and devoted her life to caring for children with life-threatening diseases. She was a remarkable person and I will miss our visits together.
